📄 Media Release Consent Form – Angel Hill / AngelHillTV

By submitting this form, you agree to the following terms:

I, the undersigned, hereby grant Angel Hill, AngelHillTV, and any affiliated brands, businesses, successors, legal representatives, or entities created now or in the future by Angel Hill (collectively referred to as “AngelHillTV and Affiliates”) the irrevocable, worldwide, royalty-free right and permission to record, use, reproduce, edit, publish, distribute, publicly display, and monetize my image, voice, likeness, and any statements made by me, in whole or in part, in video, audio, photographic, written, or digital format (collectively “Media Content”).

I understand that the Media Content may be used across any and all current or future digital or physical formats, including but not limited to:

📌  Social media platforms (e.g., Instagram, YouTube, Facebook, TikTok, X, LinkedIn)

📌  Websites, blogs, podcasts, advertising, promotional materials

📌  Streaming platforms

📌  Digital courses, online trainings, or programs

📌  Affiliate promotions and monetized content

📌  Print publications or physical event recordings

I acknowledge and agree that:

📌  I will not receive any financial compensation for the use of my image, voice, likeness, or statements.

📌  AngelHillTV and Affiliates may monetize the content in any form, including advertising revenue, paid partnerships, and affiliate earnings.

📌  I waive any right to inspect or approve the final product or its use.

📌  This release is perpetual, non-exclusive, and non-revocable, and applies retroactively and going forward to all media captured by Angel Hill in any format. 

I confirm that:

📌  I am at least 18 years of age (or have received permission from a parent or legal guardian if under 18).

📌  I am voluntarily providing this consent and understand its legal effect.
